New Out Of Hours Diabetes Services to be launched in North East Lincolnshire

“A complimentary service designed to help engage with patients who are unable to attend during the day” 

A new out-of-hours service is being launched to help diabetic patients who cannot attend clinics in working hours manage their condition. 

The service has been set up by 360 Care, a group of GP practices working together in North East Lincolnshire. This service is available to:

– Patients who need support to improve the control of their diabetes
– Patients who cannot attend during core hours
– Patients who require conversion to insulin treatment 

Clinic Times will be evenings 6.30 pm to 8.00pm or Saturday Morning 9.00am to 12.00pm. The service is available at the following Medical Centres (on a rotational basis). 

-Raj Medical Centre, Laceby Road
-Pilgrim Primary Care Centre, Immingham
-Cromwell Primary Care Centre, Cromwell Road
-Stirling Medical Centre, Stirling Street 

This service is not designed as a replacement for scheduled diabetes clinics in the surgeries but will benefit those patients who are hard to reach due to commitments such as their work shift patterns.

Attending regular clinics is important for diabetic patients as it helps them manage their condition, treatment and lifestyle more successfully, allowing them to stay healthier and reduce complications. Clinicians and other practice staff will also benefit from sharing specialist skills and the opportunity to work more closely with other health care professionals. 

Dr Arun Nayyar, General Practitioner at the Roxton Practice, said: “This service is another example of responding to the changing requirements of those patients who struggle to access routine care during working time. It demonstrates different practices working collaboratively for the benefit of local community.”
